Breast reduction surgery is usually a safe procedure when undertaken by a qualified surgeon. Nevertheless, it is not a simple surgical procedure. It requires significant disturbance to the structure of the patient’s breasts and therefore poses a degree of uncertainty and risk, as is the case with all major surgical interventions. Some of the risks and possible complications that may arise from breast reduction surgery have been described below.
1. General Risks associated with plastic surgery
There are many risks associated with plastic surgery in general which also apply to breast reduction surgery:
*Allergic reactions to anesthetic
*Negative reactions to medications (sudden rashes, difficulty in breathing, decreased/increased heart rate, hives on the skin, wheezing…etc.)
*Infection of wounds
2. Loss of sensitivity in the breast
This is fairly common as the procedure involves severing of nerve endings. Sensitivity will return with time as the nerves regenerate. Depending on the extent of the damage, this may take several months. Permanent loss of sensation may also occur but is a rare event.
3. Keloid scarring
The formation of abnormal scar tissue that spreads away from the area of the wound is called keloid scarring. This is frequent in patients who smoke.
4. Wound separation
The separation of the wound edges may occur after the surgery. The patient should keep the area very clean and contact the surgeon immediately. Revision surgery may be necessary to re-suture the incision.
5. Cosmetic issues
The patient may be dissatisfied with many aspects of the outcome of the surgery including the following:
*Breast asymmetry
*Presence of visible scars, including wide scars as a result of slow healing
*Unequally positioned nipples
*Hyperpigmentation (accumulation of blood in the tissues resulting in permanent bruising)
*Dissatisfaction with breast size
*Change in nipple color
*Boxy or flat breasts (usually temporary)
6. Hematoma
Hematoma is the pooling of clotted blood in the breast tissue. There is a 3-4% risk of this happening. It may cause the breast to swell and become painful. There may be formation of scar tissue as a result. Further surgery may be required to stop the bleeding.
7. Tissue Necrosis
This is a serious complication that both you and your surgeon must watch out for. If the fat tissue within the breast does not receive adequate blood supply (due to tissue damage), it may turn necrotic. Symptoms include an orange clear liquid draining from the wound. This needs to be treated immediately or it may give rise to a major infection and cause gangrene. This is a rare event but the risks are higher in people with impaired healing abilities, smokers, and diabetes sufferers. Hyperbaric Oxygen Treatment (HBOT) may be recommended by the surgeon and must be taken as a matter of urgency.
8. Pulmonary Thromboemboli
This refers to the formation of a blood clot in the lungs. This is a risk when liposuction forms part of the surgery. A clot may form in the breast and migrate to the lungs where it may cause cardiac arrest and coma.
